No, at this time, we are not accepting any new patients.
How can I access my medical records?
If you received care from Miga, we want to make sure you have access to your medical records. Your medical records have been securely saved in compliance with your state’s regulations and HIPAA privacy standards. To ensure continuity of care, we have chosen Clary Document Management, who will act as the custodian of your records. Copies of your records can be transferred to you or another provider with your authorization. To request this, please complete the Authorization to Release Medical Records Form.
